A patient moves between a hospital, a clinic, a pharmacy, a laboratory and a specialist office over time, and each visit leaves a record — diagnosis, prescription, lab result, vaccination or vitals — at the facility involved. Whether the rest of the network can see that record depends entirely on interoperability: facilities using a common standardized exchange format (modeled on HL7 FHIR) share data instantly, while facilities stuck on incompatible proprietary formats silently lose it.
