How it works

Every visit adds one clinical record — a diagnosis, prescription, lab result, vaccination or vitals reading — at the facility the patient walks into. Each facility only ever sees its own records for certain. Whether it also sees records created elsewhere depends on interoperability: a link between two facilities only carries data when both speak the same standardized exchange format (modeled loosely on HL7 FHIR). If either side still runs a proprietary, non-standard format, the link is incompatible and every record that would have crossed it is silently lost — the receiving doctor never learns it exists.

Green link — both endpoints use the standardized format; records flow instantly and completely.
Red link — at least one endpoint uses an incompatible proprietary format; records generated on either side never reach the other.
Profile completeness — the average, across all facilities, of how much of the patient's true record set each one can actually see.
Records lost — the running count of facility-record pairs that failed to transmit because of a format mismatch.

Real-world relevance: this is the core reason care fragments even when every clinic has "gone digital" — data silos form wherever two systems can't speak a common interoperability standard, which is exactly the problem FHIR and similar standards exist to solve.
