This is the same etiology-based receptor model as the 2D antiemetic simulator: nausea signals converge on the vomiting integration network from one of four pathways — the chemoreceptor trigger zone (CTZ, dopamine D2 / serotonin 5-HT3), the GI tract (5-HT4 / D2 via vagal afferents), the vestibular apparatus (histamine H1 / muscarinic M1) or the cortex (anxiety and raised intracranial pressure). Each antiemetic occupies its target receptor following a real Hill-Langmuir dose-occupancy curve, occupancy = dosen / (dosen + EC50n), and its clinical response is gated by how well its receptor profile matches the active pathway — exactly the mechanism-based prescribing logic from the 2D stage narrative. In 3D, the four pathway nodes sit on a real sphere around a rotating central integration hub, drug molecules travel through genuine 3D space toward the active pathway, occupied receptors appear as an instanced cluster of bound spheres scaled to the live occupancy fraction, and nausea score, vomiting frequency, antiemetic response and oral intake tolerance are rendered as a live 3D bar tower — all inspectable from any angle via free camera orbit.
