Because clearance and volume of distribution move in different directions and at different rates during development, simple weight-based scaling from adult doses is not sufficient on its own — age-stratified guidance is layered on top of weight-based dosing.
Pediatric vancomycin dosing is conventionally expressed in milligrams per kilogram of body weight, either per dose or aggregated per day, rather than as a fixed absolute dose. Age-stratified nomograms then layer an interval recommendation on top of the mg/kg figure, reflecting the age-dependent pharmacokinetic differences described in Stage 1. A weight-based nomogram typically works in two linked steps:

2. The product of dose-per-administration and number of administrations per day determines total daily exposure. Neonates — and premature infants in particular — represent the most pharmacokinetically distinct subgroup in pediatric dosing. Immature renal function substantially slows vancomycin clearance, so this population requires more conservative, typically longer, dosing intervals than older infants and children. Prematurity, postnatal age, and even day-to-day renal maturation in the first weeks of life can further modify the appropriate interval.
Renal function at birth is far from mature, and it matures at different rates depending on gestational age at delivery:

Because of this markedly reduced and rapidly-evolving clearance capacity, neonatal dosing intervals in most nomograms are longer (less frequent dosing) than those used in older infants and children, even though the mg/kg-per-dose figure may be similar or only modestly lower. This reflects a conservative approach that prioritizes avoiding drug accumulation over maximizing early exposure.
The combination of immature clearance and a narrow therapeutic margin means neonatal dosing benefits particularly strongly from early level monitoring — a conservative nomogram starting point is intentionally designed to be refined quickly once real patient data (levels) are available.
An age- and renal-function-based nomogram provides a structured, practical starting point for selecting a dosing interval before any patient-specific level data exists. Rather than requiring a full pharmacokinetic calculation for every child before the first dose, the nomogram condenses population-level developmental pharmacokinetic patterns into a simple grid: cross-reference age group (and, in more detailed nomograms, an estimate of renal function) against a recommended starting interval and mg/kg dose.
A typical pediatric vancomycin nomogram is organized as a lookup table or grid:
• Rows or bands represent age categories (e.g., neonatal, infant/young child, older child/adolescent), sometimes further subdivided by gestational or postnatal age within the neonatal band • Columns or an additional axis may represent an estimate of renal function status, where available, to further adjust the interval • Each cell of the grid specifies a recommended starting mg/kg dose and dosing interval
Clinicians select the cell matching the child's characteristics to obtain an initial dosing recommendation. This nomogram-derived starting point is deliberately conservative and standardized — it is designed to be safe and reasonable across a population, not perfectly tuned to any single child's actual drug handling.
The nomogram's core value is speed and consistency: it allows a reasonable first dose to be selected immediately, using only readily available information (age, weight, and basic renal status), without waiting for pharmacokinetic sampling.
Because a nomogram reflects population averages within each age band, it cannot fully capture the substantial inter-patient variability that exists even among children of the same age and similar weight. Factors such as individual renal function, hydration status, concurrent nephrotoxic medications, and critical illness can all shift an individual child's actual clearance away from the population-typical value the nomogram assumes.
For this reason, nomogram-guided dosing is explicitly understood as a starting point — the first step in a broader process that is completed by therapeutic drug monitoring and individualization, described in Stage 5.
As in adults, therapeutic drug monitoring (TDM) is used to refine the pediatric vancomycin dose from its nomogram-derived starting point to a regimen tailored to the individual child's actual pharmacokinetics.
