A real state-vector simulation: Hadamard, CNOT, T and Pauli gates act on actual complex qubit amplitudes. Decoherence and error correction are modelled as genuine per-gate noise on the state vector, not a cosmetic effect — fidelity and probabilities below are computed from it.
