Immunoassay drug-checking as an overdose-prevention layer — from sample to community-level impact
Fentanyl test strips (FTS) were engineered for a completely different job: screening urine for fentanyl metabolites in workplace and clinical drug testing. Around 2018, harm-reduction programs began using them off-label to test the drug supply itself — dissolving a small amount of powder, crushed pill, or cooker residue in water before use. The chemistry works the same way; the challenge is that illicit fentanyl is almost never mixed evenly through a batch.
Illicit fentanyl is typically mixed into a filler (lactose, mannitol, caffeine) or pressed into counterfeit pills using equipment never designed for pharmaceutical-grade homogeneity. The result is "hot-spotting": one corner of a pressed pill can carry a near-lethal dose of fentanyl while an adjacent corner contains almost none.
Because an FTS only samples whatever is dissolved into the test water, testing a single crumb from one spot can produce a clean-looking negative result even when the rest of the batch is heavily contaminated. Harm-reduction guidance increasingly recommends:
• Crushing the full pill (or a representative cross-section) before dissolving a portion • Testing residue left in a cooker or baggie after use, in addition to the substance itself • Treating a single negative test on a small sample as informative, not definitive
FTS are lateral-flow immunoassay strips originally cleared for detecting fentanyl metabolites in urine at nanogram-per-milliliter sensitivity — the same underlying technology used in home pregnancy tests and rapid COVID antigen tests. Harm-reduction organizations repurposed them starting around 2017–2018 after recognizing that the same antibody chemistry could detect fentanyl dissolved directly in water, before a substance is ever consumed.
This repurposing occurred faster than formal manufacturer validation for drug-checking use, but a growing body of applied chemistry studies since has confirmed that FTS reliably detect fentanyl and many analogs in drug solutions at the concentrations typically found in the illicit supply.
Testing only a pinch from one corner of a pill can create a false sense of security. Because fentanyl distributes unevenly through illicit batches, a negative result on one small sample says less about the whole supply than most users assume.
Once dipped into the dissolved sample, the test strip works entirely through passive physics and antibody chemistry — no batteries, no electronics. Capillary action wicks the liquid through a stack of engineered materials, each layer performing one step of a miniature chemical assay in under five minutes.
A fentanyl test strip is built from four bonded layers on a plastic backing card:
• Sample pad: absorbs the dissolved solution and begins wicking it forward • Conjugate pad: pre-loaded with anti-fentanyl monoclonal antibodies bound to colored gold nanoparticles, which are released and carried forward by the advancing liquid • Nitrocellulose membrane: contains two immobilized capture zones — a test line (fentanyl-conjugated antigen) and a control line (anti-species antibody) • Wicking (absorbent) pad: pulls the liquid through the membrane at a steady rate and prevents backflow
As the solution migrates across the membrane by capillary action alone, any fentanyl molecules present travel together with the antibody-gold conjugates toward the capture lines.
Unlike a sandwich-format test (such as most COVID antigen tests, where a visible line indicates a positive result), FTS use a competitive immunoassay format — the same logic used in many drug-of-abuse urine screens.
The test line carries immobilized fentanyl-analog molecules. As the sample flows past, free fentanyl in the sample and the immobilized fentanyl on the membrane both compete for the same limited pool of antibody-gold conjugates:
• If fentanyl is present in the sample, it binds up the antibody conjugates in solution before they reach the test line — few or no conjugates remain to bind there, so the test line stays blank. Result: POSITIVE (line absent). • If no fentanyl is present, antibody conjugates flow freely to the test line and bind it, producing a visible colored band. Result: NEGATIVE (line present). • The control line uses a separate antibody-antibody reaction that is unaffected by fentanyl, so it must always appear for the result to be considered valid.
Many first-time users intuitively expect "a line means positive," the opposite of how these devices actually work. Because the read logic is inverted relative to a home pregnancy test, clear instructional materials and practice reads are essential to prevent dangerous misinterpretation.
A finished strip shows one of three patterns. Reading it correctly — and understanding what it cannot tell you — is as important as running the test in the first place. FTS report presence or absence only; they were never designed to estimate dose, purity, or how many different substances are present.
POSITIVE — one line (control only): fentanyl or a cross-reactive analog was detected above the assay cutoff. This is the single most actionable result: it should change what happens next.
NEGATIVE — two lines (control + test): no fentanyl or covered analog was detected above the cutoff in this specific sample. This does not confirm the substance is fentanyl-free — only that this portion, tested this way, did not trigger the assay.
INVALID — no control line appears at all, regardless of the test line: the strip did not function correctly (insufficient sample, degraded strip, wrong solution). An invalid result must always be discarded and retested with a fresh strip — it provides no information about the sample.
FTS report presence or absence, not concentration. A strip cannot distinguish trace cross-contamination (a stray few micrograms from shared equipment) from a batch that is heavily cut with fentanyl — both read as an equally "positive" single line. This matters clinically: a positive result carries real information about risk, but it cannot be read as a dose estimate.
Cross-reactivity with the antibody's binding target is also imperfect. The antibodies are raised against fentanyl's core molecular scaffold, which most fentanyl analogs share — but structurally distinct opioids can slip past undetected. The most clinically significant blind spot is the benzimidazole ("nitazene") class — isotonitazene, metonitazene, and related compounds are potent synthetic opioids that are chemically unrelated to fentanyl and, depending on the specific strip and analog, may not reliably trigger a positive result even though their overdose risk can rival or exceed fentanyl's.
For years, FTS were legally classified as drug paraphernalia in most U.S. states — the statutes were written broadly enough to cover any device "used to test the strength or purity" of a controlled substance, sweeping up test strips originally intended for entirely different clinical purposes. Simple possession or distribution could carry criminal penalties, which discouraged both harm-reduction organizations and individual users from adopting them even as overdose deaths climbed.
Starting in 2021, momentum shifted sharply. The CDC formally endorsed fentanyl test strips as an evidence-based overdose-prevention strategy and made federal harm-reduction funds eligible to purchase them. In the years since, most U.S. states have amended their paraphernalia statutes to specifically exempt drug-checking equipment. The legal landscape nonetheless remains a patchwork — a handful of states and many countries still restrict distribution — and stigma-driven inconsistency in enforcement persists even where strips are technically legal.
A negative result reduces risk but does not eliminate it. Harm-reduction guidance is consistent on this point: treat every result — positive or negative — with the same baseline caution you would apply to an unknown supply.
A test strip only reduces overdose risk if the result changes what a person does next. Multiple U.S. harm-reduction studies since 2018 have found that a substantial majority of people who receive a positive fentanyl result report taking at least one protective action — evidence that drove the strips' rapid adoption as a frontline overdose-prevention tool.
Harm-reduction counseling built around FTS results generally promotes a small set of concrete, low-barrier actions rather than an all-or-nothing choice to use or abstain:
• Dose reduction — start with a smaller amount ("go low and slow") and wait to assess effects before using more • Never using alone — use with someone present, over a phone/video call, or through a virtual spotting hotline that can call for help if needed • Naloxone on hand — keep naloxone (Narcan) accessible and ensure at least one person present knows how to administer it • Avoiding polysubstance combinations — not mixing with alcohol, benzodiazepines, or other depressants, which compound respiratory-depression risk • Discarding or exchanging the batch — a meaningful minority of users who test positive choose not to use that supply at all • Using at a supervised consumption site, where available, so trained staff can respond immediately to an overdose
A negative FTS result can create false reassurance. Because the assay only screens for fentanyl and a defined panel of cross-reactive analogs, it says nothing about several other adulterants now common in the illicit drug supply — most notably xylazine, a veterinary sedative associated with severe wounds and its own overdose risk, and novel nitazene opioids that can fall outside the antibody's detection range. Separate test strips exist for xylazine, but they are not bundled with every FTS distribution program.
Harm-reduction messaging consistently frames a negative result as risk-reducing, not risk-eliminating — the same protective behaviors (starting low, not using alone, keeping naloxone present) are recommended regardless of what the strip shows.
FTS are a risk-reduction tool, not a guarantee of safety. Evidence shows they work best as one layer in a stack that also includes naloxone access, never using alone, and starting with a smaller dose — the small margins that most often make the difference in an overdose.
Zoomed out across a community, no single test strip prevents an overdose by itself — but aggregated across thousands of uses, paired consistently with naloxone distribution and safer-use education, fentanyl test strip programs are associated with measurably lower rates of fatal overdose in the communities that have scaled them.
Early pilot studies were the turning point for policy. A 2018 Rhode Island study (Krieger et al.) found that among people who use drugs offered FTS, the large majority used them, and roughly 81% of those who received a positive result reported changing their behavior in response — far higher than researchers had anticipated. A subsequent North Carolina study (Peiper et al., 2019) replicated the finding that FTS use was associated with self-reported protective behavior change without evidence of increased drug use.
These pilot results, combined with the escalating share of overdose deaths involving fentanyl, led the CDC to formally endorse FTS in 2021 as an evidence-based component of a comprehensive overdose-prevention strategy — a designation that unlocked federal harm-reduction funding for strip distribution nationwide. It is important to read this evidence carefully: most published studies measure self-reported behavior change, not directly observed overdose reduction, so causal claims about population-level death-rate impact should be treated as associational rather than definitively proven.
No harm-reduction tool operates in isolation, and FTS are explicitly designed to complement — not replace — the rest of the overdose-prevention toolkit:
• Naloxone distribution: reverses an opioid overdose within minutes when administered promptly; nearly every FTS program bundles naloxone kits and training alongside strips • Supervised consumption sites: allow monitored use with trained staff on hand to respond immediately if an overdose occurs, addressing the "never use alone" gap directly • Advanced drug-checking services: mass spectrometry and FTIR-based checking (available at some fixed-site programs) is quantitative and can detect adulterants — including nitazenes and xylazine — that a simple immunoassay strip misses entirely • Treatment and MAT access: medication for opioid use disorder (buprenorphine, methadone) remains the most effective long-term reduction in overdose risk; FTS-driven conversations are frequently used as a low-barrier entry point into treatment discussions
Despite rapid legalization since 2021, a patchwork of paraphernalia restrictions persists in some U.S. states and in many countries outside the U.S., and harm-reduction program funding remains politically and financially unstable year to year. Stigma continues to suppress uptake even where strips are fully legal and freely distributed.
On the technical side, no single strip currently detects xylazine, medetomidine, or other emerging non-opioid adulterants — separate strips exist for some of these, but are not universally co-distributed. And because the illicit supply continuously evolves toward new analogs, antibody panels require ongoing validation and updating to keep pace with novel nitazenes and other structurally distinct synthetic opioids entering the market.
No single tool — including fentanyl test strips — ends the overdose crisis alone. The consistent finding across studies is that FTS are a low-cost, high-leverage entry point: they increase the odds someone reaches for naloxone, chooses not to use alone, or starts with a smaller dose — the small margins that most often save a life.
