Age-related macular degeneration progresses to its "wet" (neovascular) form when retinal pigment epithelial stress and hypoxia trigger overexpression of vascular endothelial growth factor (VEGF-A). VEGF is a potent angiogenic and vascular-permeability signal — under normal conditions it supports vessel maintenance, but pathologic overproduction drives fragile, disorganized new vessels to sprout from the choroid through Bruch's membrane into the subretinal and sub-RPE space, where they leak fluid, lipid, and blood beneath the macula.
The RPE–choroid–Bruch's membrane complex normally maintains a tight barrier separating the choroidal circulation from the retina. In wet AMD:
• Chronic oxidative stress, drusen accumulation, and local hypoxia upregulate VEGF-A transcription in RPE and inflammatory cells • VEGF-A binds VEGFR-2 on choroidal endothelial cells, driving proliferation, migration, and new capillary sprouting (angiogenesis) • VEGF also directly increases vascular permeability (historically named "vascular permeability factor") by disrupting endothelial tight junctions • The resulting neovascular membrane lacks normal pericyte support and tight junctions — vessels are fragile, tortuous, and prone to leakage and rupture • Leaked fluid accumulates as subretinal fluid (SRF) or intraretinal fluid/cysts (IRF), visible on OCT as hyporeflective spaces distorting the photoreceptor layer • Recurrent microhemorrhage and fibrosis can follow, producing a disciform scar and permanent central vision loss if untreated
Because VEGF sits at the top of this cascade, pharmacologically neutralizing it — rather than only treating downstream fluid or vessels — became the therapeutic strategy that transformed wet AMD from a nearly universally blinding disease into a manageable chronic condition.
Before anti-VEGF therapy (pre-2005), roughly 90% of patients with subfoveal choroidal neovascularization lost significant vision within two years despite laser and photodynamic therapy. Anti-VEGF injections reversed this: most treated eyes now stabilize or improve vision, converting an inevitably blinding disease into a chronic, manageable one requiring lifelong surveillance and re-treatment.
Rather than dosing anti-VEGF agents systemically — which would require enormous doses to reach therapeutic retinal concentrations and would risk systemic anti-angiogenic side effects — the drug is injected directly into the vitreous cavity through the pars plana, roughly 3.5–4mm posterior to the limbus. This route bathes the vitreous and, via diffusion, the retina in drug at concentrations many-fold higher than any achievable systemic dose, while the blood-retinal barrier limits outward leakage into the systemic circulation.
The intravitreal route was adopted specifically because it maximizes retinal drug exposure while minimizing systemic risk:
Procedure sequence: • Topical or subconjunctival anesthesia, povidone-iodine antisepsis of the lid margins and ocular surface (critical for endophthalmitis prevention) • Sterile lid speculum, calipers to mark the injection site in the inferotemporal or superotemporal quadrant • 30–32 gauge needle inserted perpendicular to the sclera through the pars plana (avoiding the lens anteriorly and retina posteriorly) into the vitreous cavity • Drug volume (typically 0.05 mL) injected as a bolus; needle withdrawn, gentle pressure or spot-check IOP
Why this route dominates over systemic administration: • Blood-retinal barrier (inner: retinal vasculature tight junctions; outer: RPE tight junctions) impedes systemic drug from reaching the retina at meaningful concentration • Systemic dosing sufficient to reach the retina would carry substantial risk of impairing VEGF's physiologic roles elsewhere — wound healing, renal glomerular function, systemic vascular homeostasis • Direct intravitreal delivery achieves retinal drug concentrations 1000-fold or more above what systemic dosing could safely provide • Vitreous acts as a local drug depot: the gel matrix slows clearance, extending the duration of therapeutic effect compared to an aqueous compartment
Agents in routine use (ranibizumab, aflibercept, bevacizumab off-label, faricimab, brolucizumab) share this delivery route despite differing molecular size, VEGF-binding affinity, and half-life — the intravitreal injection itself is the common pharmacokinetic gateway for all of them.
Once inside the vitreous, the anti-VEGF molecule diffuses toward the retina and binds circulating and tissue-bound VEGF-A with high affinity, sequestering it before it can engage VEGFR-2 on choroidal endothelial cells. With the angiogenic and permeability signal neutralized, the leaky neovascular complex quiets: vascular permeability falls, existing fluid is gradually resorbed, and further endothelial proliferation is suppressed — producing the anatomic drying and visual stabilization seen on follow-up OCT within days to weeks of injection.
Three main anti-VEGF drug classes are used clinically, each neutralizing VEGF by a distinct molecular architecture:
• Ranibizumab (Lucentis): a humanized Fab antibody fragment (48 kDa), engineered for high affinity and small size to improve retinal penetration; binds all biologically active VEGF-A isoforms • Aflibercept (Eylea): a fusion protein ("VEGF-Trap") combining VEGFR-1 and VEGFR-2 binding domains with an IgG Fc region; acts as a soluble decoy receptor with picomolar affinity, also binds VEGF-B and placental growth factor • Bevacizumab (Avastin, off-label): a full-length humanized IgG1 antibody (149 kDa), originally developed for systemic oncology use, repurposed intravitreally at a fraction of its systemic dose • Faricimab: a bispecific antibody neutralizing both VEGF-A and angiopoietin-2, targeting vascular destabilization through a second pathway • Brolucizumab: a small single-chain antibody fragment (26 kDa) enabling high molar dose per injection
Once bound, the anti-VEGF–VEGF complex is pharmacologically inert — VEGF can no longer engage VEGFR-2, halting the downstream signaling (PLCγ, PI3K/Akt, Ras/MAPK) that drives endothelial proliferation, migration, and tight-junction disruption. Downregulated permeability allows fluid resorption by the RPE pump and choroidal circulation, visible as progressive anatomic drying on serial OCT.
Anatomic response and visual response do not always move together: fluid can resolve on OCT while visual acuity gains plateau, particularly in eyes with chronic photoreceptor damage or fibrosis predating treatment. This is why treatment decisions weigh both anatomic (OCT fluid) and functional (visual acuity) response, not fluid status alone.
No anti-VEGF agent remains in the eye indefinitely. After injection, the drug is cleared from the vitreous primarily via the anterior route (diffusion into the aqueous humor and outflow through the trabecular meshwork) with a smaller posterior contribution across the blood-retinal barrier. As vitreous concentration falls below the threshold needed to keep free VEGF fully neutralized, VEGF activity gradually resumes — which is why suppression is inherently time-limited and re-injection, not a single dose, is the standard of care.
Vitreous drug concentration follows approximately first-order (exponential) decay after the injection bolus:

• Vitreous half-life differs modestly among agents (roughly 5–11 days) — but the clinically relevant "durability" of VEGF suppression is longer than the molecular half-life alone would suggest, because binding affinity and molar dose determine how far concentration must fall before free VEGF re-accumulates above the pathologic threshold • Aflibercept's very high VEGF-binding affinity and receptor-decoy design let it maintain adequate suppression for longer despite a vitreous half-life similar to ranibizumab — illustrating that potency, not half-life alone, determines durability • Newer agents (faricimab, high-dose aflibercept 8mg, brolucizumab) were specifically engineered to extend the durability window, permitting longer intervals between injections without losing disease control • Once free (unbound) VEGF concentration climbs back above the level needed to drive VEGFR-2 signaling, endothelial permeability and proliferative activity resume — often preceding any visible recurrence of fluid on OCT by days • This underlies why "treat and observe" (PRN dosing based on symptoms alone) tends to underperform proactive fixed or treat-and-extend regimens in trials — recurrent fluid and vision loss can occur between visits if monitoring is infrequent
Because suppression wanes over weeks, every anti-VEGF regimen must define how often to re-inject. No single interval fits all patients: some eyes need monthly dosing indefinitely, others sustain control on quarterly injections. Three broad strategies balance disease control against injection burden, office visits, and cumulative treatment risk — the choice, and its ongoing adjustment, is individualized to each eye's observed recurrence pattern.
1. Fixed interval (e.g., monthly or bimonthly): • Every patient re-injected on a rigid schedule (commonly every 4 or 8 weeks) regardless of individual disease activity • Maximizes suppression and vision outcomes in trials (e.g., ANCHOR, MARINA) but imposes the highest injection and visit burden • Often used for loading doses (e.g., 3 monthly injections) at treatment initiation before transitioning to an individualized strategy
2. Treat-and-extend (TAE): • After disease is quieted with initial monthly dosing, the interval between injections is progressively lengthened (typically by 2-week increments) as long as the eye remains free of fluid/hemorrhage at each visit • If recurrence is detected, the interval is shortened back toward the last stable interval • Every visit includes both an examination/OCT and an injection — no "wait and see" gap — which proactively prevents the vision loss that can occur between a recurrence and its detection • Converges on a personalized maintenance interval (commonly 8–16 weeks) reflecting that eye's own VEGF suppression durability
3. As-needed / pro re nata (PRN): • Patient is monitored on a fixed visit schedule (e.g., monthly) but injected only when recurrent fluid, hemorrhage, or vision loss is documented • Reduces total injection count versus fixed dosing, but requires strict adherence to monitoring visits, since vision loss can occur in the interval before a recurrence is caught • Real-world outcomes with PRN are more variable than TAE, largely driven by inconsistent monitoring adherence
Selection depends on lesion characteristics, fellow-eye history, response to loading doses, adherence capacity, and shared decision-making — with ongoing re-evaluation as each eye's recurrence pattern becomes clearer over successive visits.
Treat-and-extend has become the dominant real-world strategy in most retina practices because it matches the proactive philosophy of fixed dosing (never letting the eye go untreated once fluid is confirmed) with the reduced burden of individualized intervals — most eyes settle into a personal maintenance rhythm within the first year, meaningfully reducing lifetime injection counts without sacrificing vision outcomes.
