A blockage partway down the ureter (commonly a lodged kidney stone) doesn't stop the kidney from producing urine — it stops that urine from draining. The trapped fluid backs up into the kidney's collecting system, which distends (hydronephrosis) as more volume accumulates behind the block. Rising volume raises pressure inside the collecting system, and that pressure presses outward on the kidney tissue itself, throttling its blood supply.
Tissue damage here accumulates over time whenever pressure sits above a safe threshold — so a short episode, even a fairly complete blockage, can resolve with little lasting harm, while a lower-grade partial blockage left uncorrected for days or weeks can rack up comparable or greater cumulative damage. Both severity and duration drive the outcome independently.
