How it works

A laparoscopic instrument is a rigid rod that passes through a fixed point — the trocar in the abdominal wall. Because that point cannot move sideways, the entire instrument behaves as a rigid body rotating about a single fulcrum, exactly like a see-saw or a lever pinned at one point.

Fulcrum condition:  handle, pivot and tip stay collinear
For a small hand tilt θ about the trocar:
  external (hand) lateral travel  ≈ L_out · sin(θ)
  internal (tip)  lateral travel  ≈ L_in  · sin(θ)
  tip moves in the OPPOSITE direction to the hand
  motion-scale factor = L_in / L_out

Insertion depth sets L_in (trocar to tip) and, since total shaft length is fixed, L_out (trocar to hand) shrinks to compensate.
Hand tilt sliders rotate the whole rigid instrument about the fixed trocar point — this is the only degree of freedom a real trocar allows besides in/out sliding and rotation about the shaft's own axis.
When L_in < L_out (shallow insertion, long external segment) the tip's motion is attenuated — small, precise tip movements from larger hand movements.
When L_in > L_out (deep insertion, short external segment) the tip's motion is amplified — small hand tremor becomes large tip motion, which is why surgeons brace their hands and use shorter strokes deep in the abdomen.
The mirrored direction is why novice laparoscopic surgeons must learn to move the hand left to send the tip right — a skill explicitly drilled in laparoscopic box-trainer curricula.

Real-world relevance: this fulcrum (paradoxical-motion) effect is one of the first perceptual-motor challenges taught in minimally invasive surgery training, alongside loss of depth perception and haptic feedback through the instrument shaft.
