Immunoassay fentanyl test strips (dip a strip in a dissolved sample, wait for a line) are cheap, fast, and have meaningfully reduced overdose risk since their wide adoption in the mid-2010s. But they are binary: they answer only "is fentanyl present, yes or no" above a fixed detection threshold.

A typical submission involves: the client scrapes or portions a small amount of the substance (residue from a baggie, a shaving from a pressed pill, or drug-cooker residue), places it in a provided container, and completes a brief, optional, anonymous intake form (expected substance, route of use, geographic area of purchase — never name or contact info unless the client opts into a callback).
Some programs offer walk-up storefront checking with a wait-while-you-watch turnaround; others use mail-in or drop-box models feeding a centralized lab, trading speed for wider geographic reach. Mobile/outreach checking meets people where they already receive other harm-reduction services (syringe access, naloxone distribution).
The sample is placed into an analytical instrument that probes its molecular structure directly. Fourier-transform infrared (FTIR) spectroscopy and mass spectrometry are the two workhorse technologies — each generates a detailed, quantifiable signature of every compound in the sample, not just a reaction to one pre-chosen target.
FTIR works by shining a broad band of infrared light through (or reflecting it off) the sample and measuring which wavelengths are absorbed. Because fentanyl is active at microgram doses, it can be present at well under 1% of a sample's total mass — below reliable FTIR detection in some mixtures. Many programs pair FTIR with an immunoassay strip specifically to catch this blind spot.
Mass spectrometry (often paired with gas or liquid chromatography as GC-MS or LC-MS) first physically separates the sample into its component compounds, then ionizes each and measures its mass-to-charge ratio. The resulting fragmentation pattern is compared against reference libraries to identify compounds with very high confidence, even at trace concentrations — down to nanogram or part-per-billion sensitivity.
This sensitivity is exactly what is needed to reliably detect fentanyl and its analogs, nitazenes, and other potent synthetic opioids present at very low relative concentration in a bulked sample. The raw output of either instrument is just a spectrum — a set of peaks with no inherent labels. Turning that into an actionable identification requires:

A spectrum alone is not an answer — it becomes one only when matched against a reference library and interpreted by software or a trained analyst. This stage identifies the primary substance, estimates rough potency, and — critically — surfaces unexpected adulterants that a simple test strip would never have been asked to look for.
Xylazine is a veterinary sedative (an alpha-2 adrenergic agonist, not an opioid) that has spread rapidly through the illicit fentanyl supply in many regions, earning the street name "tranq." It is typically added as a cutting agent believed to extend the duration of a fentanyl high.
Because xylazine is not an opioid, naloxone (Narcan) has no effect on it — a person overdosing on a fentanyl-xylazine mixture may not fully respond to naloxone even after the opioid component is reversed, requiring supportive care (airway management, oxygen) for the sedative effects. Xylazine is also associated with severe, necrotic skin wounds at injection sites, a distinct harm profile from opioid overdose alone.
An immunoassay fentanyl test strip cannot detect xylazine at all — it was never designed to. Only broader-spectrum chemical analysis (FTIR or mass-spec matched against an updated reference library) reveals its presence, which is precisely why simple test strips, while valuable, are not sufficient on their own in a supply this contaminated.
In several major US drug-checking programs, xylazine went from a rare finding to present in a majority of fentanyl samples within about two years — a shift that would have gone completely undetected by test-strip-only surveillance.
Nitazenes are a class of synthetic opioids originally developed (but never approved) as analgesics in the 1950s, now re-emerging as illicit adulterants. Some nitazene analogs (such as isotonitazene or metonitazene) are estimated to be as potent as, or more potent than, fentanyl itself — meaning even smaller trace amounts can be lethal.
Because nitazenes are structurally distinct from fentanyl, standard fentanyl test strips do not reliably detect them, and even some early-generation multi-drug immunoassay panels missed them. An identification result only saves a life if it reaches the person who needs it, in a form they can act on. Every individual result is also a single data point. Aggregated across many samples, anonymized results reveal supply-wide trends — a spike in a novel adulterant, a sudden jump in potency, a new compound never seen before in the region — that get pushed out as public alerts to the wider community, other harm-reduction services, and sometimes public health authorities.
A single unusual finding could be noise — a one-off contaminated bag. A pattern across dozens of independent samples from different clients, submitted over days or weeks, is a signal. Aggregation strips identifying detail and pools results by rough geography and time window, then applies simple trend detection: has the rate of xylazine-positive samples in this area jumped in the last two weeks? Has a compound never seen before in this region now appeared in five separate submissions?
This converts drug checking from a purely individual service into a form of real-time public health surveillance — often described as a "sentinel" system because it can detect emerging threats faster than traditional lagging indicators like overdose death records, which can take weeks to months to compile and report.

Some regions have built formal real-time alert networks connecting multiple drug checking sites, EMS overdose-spike data, and hospital toxicology reporting into a single coordinated early-warning system, so a signal detected at one checking site can trigger alerts across an entire metro area or state within hours.
Aggregate drug-checking data does not stop at public alerts — it also informs: naloxone distribution planning (stocking more, or reformulated higher-dose naloxone, in areas showing rising potency), outreach targeting (directing mobile services toward neighborhoods showing new adulterant patterns), clinical practice (emergency departments and treatment programs adjusting protocols when a region sees a xylazine or nitazene surge), and policy/funding conversations (demonstrating concrete, current local need to funders and regulators).
Drug checking is most effective as one integrated piece of a broader harm-reduction service continuum — alongside syringe service programs, naloxone distribution, overdose prevention sites, and low-barrier treatment access — rather than a standalone intervention.
