Deprescribing is the planned, supervised process of dose reduction or discontinuation of a medication that may be causing harm or no longer providing benefit — most critical in older adults, where polypharmacy, altered pharmacokinetics, and cumulative anticholinergic/sedative burden raise the risk of falls, cognitive impairment, and hospitalization. The process begins not with a taper schedule, but with a structured decision about whether continued therapy is still justified.
Aging changes both pharmacokinetics and pharmacodynamics: reduced hepatic and renal clearance prolongs drug half-lives, decreased lean body mass and increased fat alter volume of distribution for lipophilic drugs (like benzodiazepines), and the aging brain is more sensitive to sedative and anticholinergic effects at the same serum concentration that was well-tolerated decades earlier.
Medications that were appropriately started for an acute problem — a short course of benzodiazepine for post-operative anxiety, a PPI for stress ulcer prophylaxis during a hospital stay, a corticosteroid taper that never quite finished — often persist for years without re-evaluation. Deprescribing formalizes the process of asking, on a recurring basis: does the benefit of this drug still outweigh the risk, given the patient in front of us today?
The STOPP/START and Beers Criteria are the two most widely used explicit screening tools for identifying potentially inappropriate medications in older adults, and both flag long-term benzodiazepines, opioids, PPIs, and unsupervised chronic corticosteroids as priority deprescribing targets.
Before designing a taper, clinicians confirm: the original indication (is it still present?), duration of use (longer use generally requires a slower taper), current dose and formulation, prior taper or discontinuation attempts and how they went, and the patient's goals and fears about stopping.
A key principle: never stop these drug classes abruptly in a stable long-term user. Corticosteroids suppress the hypothalamic-pituitary-adrenal (HPA) axis; benzodiazepines and opioids produce physical dependence with predictable, sometimes dangerous withdrawal syndromes; and PPIs cause rebound acid hypersecretion. The starting dose becomes the anchor point (100%) from which a proportional, individualized taper schedule is built.
Successful deprescribing is collaborative, not unilateral. Patients who have taken a benzodiazepine nightly for a decade often fear rebound insomnia or anxiety more than they value the theoretical benefits of stopping. Framing the taper as slow, reversible, and patient-paced — with an explicit plan to slow down or pause if symptoms emerge — improves both adherence and outcomes.
Documentation at baseline should include validated symptom scales relevant to the drug class (e.g., Clinical Institute Withdrawal Assessment for benzodiazepines, Clinical Opiate Withdrawal Scale for opioids) so that changes during the taper can be objectively tracked against this starting point.
The shape of the taper curve matters as much as its overall length. A linear taper — cutting the same fixed mg amount at every step — produces progressively larger relative (percentage) reductions as the dose gets smaller, because pharmacological effect at receptors (especially GABA-A for benzodiazepines) follows a hyperbolic, not linear, dose-response relationship. Hyperbolic tapering keeps each step's relative reduction roughly constant, producing much gentler cuts near the end of the taper — exactly where withdrawal risk concentrates.
Consider a benzodiazepine dose-response curve: because GABA-A receptor occupancy rises steeply at low doses and plateaus at higher doses, a drop from 100% to 90% of the original dose removes only a small fraction of the drug's receptor occupancy — the patient barely notices. But a drop from 20% to 10% of the original dose, the same 10-percentage-point cut, removes a much larger share of remaining receptor occupancy. Equal absolute (linear) reductions therefore translate into escalating relative effect-size reductions as the dose falls, concentrating withdrawal risk in the final stretch of a taper — precisely where patients using a naive linear schedule are most likely to experience a crisis.
Hyperbolic tapering — reducing the dose by a fixed proportion of the current dose (not the original dose) at each step — keeps the relative pharmacological impact of every step approximately equal, which is why smaller and smaller absolute reductions are recommended as the dose approaches zero.
General guidance across drug classes converges on reducing the dose by roughly 10–25% of the current (not original) dose every 1–2 weeks, then slowing further — often to 5–10% reductions every 2–4 weeks — for the final quarter of the taper. For chronic, high-dose, or long-duration use (especially benzodiazepines used for years), tapers frequently extend over many months to more than a year.
CDC opioid guidance (2022) recommends individualized, patient-paced tapers, often 10% per month for long-term users, with even slower rates (5% per month or less) for patients on high doses or with significant comorbidity — reflecting the same hyperbolic principle: go slower as the dose gets lower.
Practical taper design also accounts for the drug's half-life: switching a short-acting benzodiazepine (e.g., alprazolam) to a longer-acting equivalent (e.g., diazepam) before tapering can smooth out peak-trough fluctuations that otherwise trigger inter-dose withdrawal symptoms. Liquid formulations, compounded capsules, or dose-splitting allow the very small final reductions that hyperbolic tapering calls for — reductions too small to achieve by tablet-splitting alone.
The taper is never rigid: if withdrawal symptoms emerge, the standard response is to hold the current dose (not reverse progress) until symptoms settle, then resume at a slower rate — a principle summarized clinically as "go low, go slow."
Withdrawal is the physiological consequence of physical dependence: the body has adapted to the drug's presence, and removing it — especially too quickly — unmasks compensatory changes that were being suppressed. Each drug class produces a distinct withdrawal syndrome with its own timeline, symptom profile, and worst-case severity, which is why taper speed must be matched to drug-specific withdrawal danger, not treated as one-size-fits-all.
Benzodiazepine withdrawal ranges from tremor, anxiety, insomnia, and sweating in mild cases to seizures, delirium, and (rarely) death in severe abrupt cessation of high-dose, long-term use — this is one of the few withdrawal syndromes considered potentially life-threatening, alongside alcohol withdrawal. Onset and course depend heavily on the drug's half-life: short-acting agents (alprazolam) produce faster, sharper withdrawal than long-acting agents (diazepam).
Opioid withdrawal is rarely life-threatening in otherwise healthy adults but is intensely distressing: myalgia, abdominal cramping, diarrhea, piloerection, yawning, and severe anxiety, tracked clinically with the Clinical Opiate Withdrawal Scale (COWS). Older adults with cardiovascular disease are more vulnerable to the autonomic surge (tachycardia, hypertension) that accompanies opioid withdrawal.
Never abruptly discontinue a long-term, high-dose benzodiazepine — unlike most withdrawal syndromes, benzodiazepine (and alcohol) withdrawal can progress to seizures and is a recognized medical emergency in vulnerable patients.
Exogenous corticosteroids suppress the HPA axis by providing negative feedback that reduces the body's own cortisol production. With prolonged use (generally more than 3 weeks, or shorter courses at very high doses), the adrenal glands can become unable to mount an adequate stress response if the drug is stopped too quickly — producing adrenal insufficiency: fatigue, hypotension, nausea, and in severe cases, an adrenal crisis with cardiovascular collapse.
Steroid tapers must therefore track both symptom relief and physiologic HPA recovery, sometimes confirmed with morning cortisol or ACTH stimulation testing before the final discontinuation step, particularly in elderly patients with reduced physiologic reserve.
The core principle of withdrawal risk monitoring is that taper speed and drug withdrawal severity multiply, not simply add: a moderately fast taper of a low-severity drug (like most PPIs) is usually well tolerated, while the same fast taper applied to a high-severity drug (long-term benzodiazepine, high-dose opioid) can trigger a dangerous withdrawal cascade.
Structured monitoring — validated symptom scales, scheduled check-ins, and a pre-agreed plan to pause or slow the taper if symptoms exceed a threshold — allows clinicians to catch rising withdrawal risk early and adjust before it becomes a crisis, rather than reacting only after severe symptoms appear.
Rebound is often confused with withdrawal, but the two are mechanistically distinct: withdrawal symptoms arise from removing a drug the body has adapted to, while rebound is a resurgence — sometimes an overshoot — of the original condition the drug was treating. Rebound can occur even in patients with minimal physical dependence, and it is a leading reason patients (and prescribers) prematurely abandon an otherwise well-designed taper.
Proton pump inhibitors suppress gastric acid so effectively that the stomach compensates by elevating serum gastrin (hypergastrinemia), which drives enterochromaffin-like (ECL) cell hyperplasia and increased acid-producing capacity. When the PPI is stopped — especially abruptly after long-term use — this adapted, hyperactive acid-secretory machinery is suddenly unopposed, producing rebound acid hypersecretion: heartburn and dyspepsia often worse than the patient's original symptoms, even in people who did not have significant reflux disease to begin with.
This rebound typically resolves within weeks to a couple of months as gastrin levels and ECL cell mass normalize, but it frequently convinces patients (and sometimes clinicians) that the PPI is "still needed," undermining an otherwise appropriate deprescribing attempt.
A step-down approach — reducing PPI dose or switching to on-demand/H2-receptor-antagonist bridging therapy before full discontinuation — substantially reduces the intensity of rebound acid hypersecretion compared with abrupt cessation.
In benzodiazepine tapers, rebound anxiety or insomnia can appear as dose reductions proceed — a resurgence of the condition the drug was originally treating, sometimes with an "overshoot" beyond baseline severity for a period before settling. This is distinct from withdrawal symptoms like tremor or autonomic hyperarousal, though the two frequently overlap and can be difficult to distinguish clinically.
Similarly, opioid tapers in patients with chronic pain can produce a transient increase in pain intensity that is not simply "untreated pain returning" but a rebound phenomenon related to central sensitization changes — important to distinguish because it often improves with time and non-opioid strategies, rather than indicating the taper should be reversed.
The clinical challenge is that rebound symptoms can look identical to "the disease coming back for good," tempting patients and prescribers to restart the drug at the original dose rather than persist through a transient flare. Anticipatory counseling — explaining that a temporary worsening near the end of the taper is expected and usually self-limited — combined with symptomatic support (e.g., H2 blockers during PPI taper, non-pharmacologic anxiety strategies during benzodiazepine taper) helps patients and clinicians ride out the rebound window without abandoning a taper that is otherwise working.
If symptoms persist well beyond the expected rebound window (weeks, not months) or clearly exceed a transient flare, re-evaluating whether the original indication has genuinely returned is appropriate — rebound is a plausible explanation, not an automatic one.
Reaching zero dose is not the end of the deprescribing process — it is the beginning of a monitoring period that confirms the discontinuation is durable. Elderly patients in particular benefit from scheduled follow-up checkpoints that catch delayed withdrawal, late rebound, or disease recurrence before it escalates, and that reinforce the gains achieved: reduced fall risk, improved cognition, fewer drug interactions, and lower pill burden.
A successful taper schedule includes explicit follow-up checkpoints after the final dose — commonly at 1 week, 4 weeks, 12 weeks, 6 months, and 12 months — to screen for delayed withdrawal symptoms (which can emerge later than expected, especially with long half-life drugs), late rebound of the original condition, and any signs that the medication is genuinely still needed.
Each checkpoint should reassess the validated symptom scale used at baseline, confirm no new safety concerns (falls, cognitive changes, mood symptoms), and document the sustained benefit of discontinuation — this creates a durable record that supports the decision if the topic of restarting the medication arises later.
Discontinuation is considered successful not merely when the dose reaches zero, but when the patient remains stable through the full follow-up window without needing to restart therapy — the taper schedule and the monitoring plan are equally essential parts of deprescribing.
Successful deprescribing of sedative-hypnotics and opioids in older adults is associated with reduced fall and fracture risk, improved cognitive clarity and daytime alertness, fewer drug-drug interactions, and simplified medication regimens that improve adherence to remaining necessary therapies. For PPIs, discontinuation avoids long-term associations under investigation (fracture risk, enteric infection, nutrient malabsorption) without sacrificing symptom control when appropriately bridged.
For corticosteroids, successful taper to physiologic replacement or full discontinuation avoids cumulative steroid toxicity — osteoporosis, hyperglycemia, cataracts, skin fragility — that disproportionately affects older adults.
Not every taper reaches zero, and that is an acceptable outcome: some patients stabilize at a lower dose that balances benefit and risk better than either extreme. If withdrawal or rebound symptoms recur despite a well-paced taper, holding at the lowest tolerated dose — rather than reflexively returning to the original dose — preserves most of the deprescribing gain while respecting the patient's tolerance.
The overall message for clinicians and patients: deprescribing is a structured, reversible, monitored process — not an irreversible gamble — and a hyperbolic, individualized, symptom-guided taper gives the best chance of reaching a safe, sustained discontinuation.
