Central line insertion bundle checklist — the five evidence-based elements that, together, prevent central line-associated bloodstream infections
Of the five elements in the central line insertion bundle, hand hygiene is by far the easiest to perform and, paradoxically, the one most often rushed or skipped entirely under time pressure. Yet contaminated hands remain one of the most direct routes by which skin and environmental organisms reach the catheter insertion site, the guidewire, or the catheter hub during placement.
Every other element of the insertion bundle — barrier precautions, skin antisepsis, site selection — can be performed flawlessly and still be undermined if the operator's hands carry transient flora onto a glove, a drape edge, or an exposed hub during setup.
Hand hygiene before the procedure serves two purposes:
1. It reduces the bioburden on the operator's own hands before donning sterile gloves, lowering the chance that a glove breach (small, often unnoticed tears occur in up to 10-15% of procedures) introduces organisms directly onto the sterile field.
2. It sets the behavioral tone for the rest of the procedure — teams that visibly perform and verbally announce hand hygiene as "step one" of a checklist show measurably higher compliance with every subsequent bundle element. This is the basis for the checklist-and-observer model pioneered by Peter Pronovost's central line bundle work in Michigan ICUs (the "Keystone Project"), which cut CLABSI rates by roughly two-thirds across more than 100 ICUs.
Technique matters as much as timing: WHO's "five moments" method — palms, backs of hands, between fingers, backs of fingers/knuckles, thumbs, and fingertips — must be completed within the ≥20 second window. A 5-second rinse under a faucet, or an alcohol rub wiped off before it dries, provides negligible antimicrobial benefit despite "looking" like hand hygiene occurred.
The Michigan Keystone ICU Project (Pronovost et al., NEJM 2006) demonstrated that a simple five-element checklist — starting with hand hygiene — reduced CLABSI rates from a median of 2.7 per 1,000 catheter-days to zero in the median ICU within three months, sustained over 18 months of follow-up across 103 ICUs.
Maximal sterile barrier precautions require every component together: a cap covering all hair, a surgical mask covering nose and mouth, a sterile gown, sterile gloves, and a large sterile drape covering the patient from head to toe with only a small fenestration at the insertion site. Sterile gloves alone — the minimum many clinicians once considered adequate — leave the rest of the operator and the patient's body as potential contamination sources.
The evidence for maximal sterile barrier precautions comes from a series of studies led by Issam Raad and colleagues in the 1990s, comparing catheter-related bloodstream infection rates between two insertion techniques:
• Control group: sterile gloves + small fenestrated drape only (the older "minimal barrier" standard) • Intervention group: cap + mask + sterile gown + sterile gloves + large full-body sterile drape
The maximal barrier group had a more than six-fold reduction in catheter colonization and roughly a two-thirds reduction in catheter-related bloodstream infections. This single change — adding a cap, mask, gown, and full-body drape to a procedure that many clinicians already considered "sterile" — became one of the most cost-effective interventions in modern hospital infection control.
Each barrier component addresses a distinct contamination route: • Cap: prevents hair and scalp flora shedding onto the field • Mask: prevents respiratory droplets and oral flora contaminating the site during conversation • Sterile gown: covers the operator's clothing and forearms, which harbor far more bacteria than freshly washed hands • Sterile gloves: direct contact barrier at the point of manipulation • Full-body drape: prevents contact contamination from the patient's own skin and bedding outside the immediate insertion field, and gives the operator a stable, unambiguous sterile working surface
A fenestrated drape covering only the insertion site — rather than the patient's entire body — was standard practice for decades and is still sometimes used today. The randomized evidence is unambiguous: full-body draping is not a cosmetic upgrade, it independently predicts lower infection rates even when every other bundle element is held constant.
Chlorhexidine gluconate in alcohol solution (typically 2% CHG / 70% isopropyl alcohol) is the preferred skin antiseptic for central line insertion, replacing povidone-iodine in most modern protocols due to its faster onset, longer residual activity, and superior reduction in catheter colonization. But the antiseptic only works if it is applied correctly and given time to fully air-dry before the needle passes through skin.
Correct chlorhexidine application follows a back-and-forth or concentric-circle friction scrub, starting at the intended puncture site and working outward, covering an area large enough to accommodate the full sterile drape fenestration plus a safety margin (typically covering a 10cm+ radius from the planned site).
The antiseptic must be allowed to air-dry completely — not blotted, fanned, or wiped away — before the skin is punctured. Alcohol is what provides the immediate, fast-acting kill; chlorhexidine gluconate provides the persistent, residual antimicrobial film that continues suppressing skin flora regrowth for hours afterward. Puncturing before the solution dries:
1. Drives incompletely-dried antiseptic and residual skin flora directly into the puncture tract on the needle or catheter 2. Denies the alcohol component time to achieve its full log-reduction of surface organisms 3. Can trap moisture under the drape, which paradoxically supports bacterial regrowth
A 2002 meta-analysis (Chaiyakunapruk et al.) comparing chlorhexidine to povidone-iodine antisepsis across multiple central-line trials found chlorhexidine associated with roughly a 50% relative reduction in catheter-related bloodstream infection — a difference attributable to both chlorhexidine's intrinsic residual activity and its more forgiving one-step application compared to iodine's multi-step paint-and-dry-and-remove protocol.
Chlorhexidine allergy or sensitivity, while uncommon, does occur — alternative antiseptics (povidone-iodine, or aqueous chlorhexidine without alcohol for very low-birth-weight neonates) should be available. But for the vast majority of adult patients, alcoholic chlorhexidine with full dry time is the standard of care.
Where the catheter is placed matters nearly as much as how carefully it is placed. In most adult patients, the femoral vein carries a meaningfully higher risk of catheter-related bloodstream infection than the subclavian or internal jugular veins, largely because of its proximity to groin flora, higher local skin moisture and temperature, and greater difficulty maintaining a clean, dry dressing in that anatomical region.
Central venous catheters can be placed in three principal sites, each with a distinct risk-benefit profile:
Subclavian vein: generally the lowest infection risk of the three sites in adult patients, due to its location away from oral and groin flora and its compatibility with a stable, easily maintained dressing. Trade-off: higher mechanical complication risk during insertion (pneumothorax, subclavian artery puncture) compared to ultrasound-guided internal jugular access, and it is not compressible if bleeding occurs — a relative contraindication in patients with coagulopathy.
Internal jugular vein: an excellent alternative, particularly with real-time ultrasound guidance, which has sharply reduced mechanical complication rates. Infection risk is intermediate — higher than subclavian but still meaningfully lower than femoral, especially with careful dressing management given its proximity to the neck and oral cavity.
Femoral vein: reserved for situations where the other two sites are contraindicated, inaccessible, or during emergent resuscitation when rapid access is prioritized over long-term infection risk. Its proximity to the groin — an area with higher bacterial density, higher skin moisture, and greater difficulty keeping a dressing clean and adherent — drives its elevated CLABSI risk. It also carries a higher risk of catheter-associated deep vein thrombosis.
CDC and SHEA guidelines both recommend avoiding the femoral site in adult patients whenever a clinically equivalent alternative exists, and encourage prompt conversion to a non-femoral site once the patient is stabilized if femoral access was used emergently.
Site selection is not simply "always avoid femoral" — in obese patients, in patients with bleeding disorders where subclavian puncture is unsafe, or during emergent access, femoral placement may be the correct clinical choice. The bundle element is about deliberate, informed site selection weighing infection risk against procedural risk — not a rigid prohibition.
The first four bundle elements reduce the risk of infection at the moment of insertion. The fifth element addresses a different, equally important risk driver: every additional day a central line remains in place is another day of cumulative CLABSI exposure. A documented daily assessment of whether the line is still clinically necessary — with prompt removal when it is not — is what prevents "temporary" access from silently becoming a weeks-long, unnecessary infection risk.
Central lines are frequently placed under urgent circumstances — for vasopressors, difficult access, or rapid volume resuscitation — and just as frequently left in place well past the point of clinical need, simply because removing a working line requires an active decision while leaving it in requires none. This asymmetry is the core problem the fifth bundle element is designed to solve.
Daily necessity review typically takes the form of a structured question posed during multidisciplinary rounds: "Does this patient still need central venous access, and does it still need to be this line, in this location?" Common findings that should prompt removal include:
• The original indication (vasopressors, TPN, frequent blood draws, lack of peripheral access) has resolved • The patient can now tolerate peripheral IV access or a PICC line for the remaining treatment course • The line has been in significantly longer than the original clinical plan anticipated, with no clear ongoing justification documented • Signs of local site complication (erythema, drainage, tenderness) that warrant reassessment regardless of continued need
Nurse-driven and pharmacist-driven removal protocols — where non-physician staff are empowered to flag or even remove lines meeting pre-defined "no longer necessary" criteria without waiting for a physician order — have independently demonstrated reductions in average catheter dwell time and corresponding CLABSI rates in multiple quality-improvement studies.
The Michigan Keystone Project bundle explicitly included daily review of line necessity as its fifth element alongside hand hygiene, maximal barrier precautions, chlorhexidine antisepsis, and optimal site selection — and attributed part of its sustained, multi-year CLABSI reduction specifically to shortened average catheter dwell time, not insertion technique alone.
