A structured, detail-oriented allergy history is the single most powerful and lowest-cost tool in the delabeling pathway: it alone can safely reclassify low-risk patients for direct oral challenge without any skin testing, while flagging the small minority whose reaction pattern makes any re-exposure testing absolutely contraindicated.
Most penicillin allergy labels originate in childhood, often from a viral exanthem coincident with an antibiotic course, a non-specific GI symptom, or a reaction to a co-administered drug. True IgE-mediated penicillin hypersensitivity is rare and, when present, wanes over time as specific IgE titers decline. Cross-sectional studies consistently find that fewer than 5% of labeled patients have a reproducible immediate reaction when systematically tested — meaning the label persists almost entirely on parental recall or a single undocumented note from decades earlier, not on any confirmatory allergy workup.
The clinical cost of a false label is substantial and frequently underestimated: labeled patients are preferentially treated with broader-spectrum, less effective, and more toxic alternative antibiotics — fluoroquinolones, vancomycin, clindamycin — driving worse outcomes, higher resistance selection pressure, and higher cost per admission.

Key elements captured on history: exact drug and formulation, time from dose to reaction onset (immediate <1h suggests IgE-mediated; delayed >72h suggests T-cell mediated), morphology (isolated GI symptoms vs. urticaria/angioedema vs. anaphylaxis vs. blistering/desquamating rash), mucosal involvement, systemic organ involvement (fever, lymphadenopathy, hepatic/renal derangement), and whether the patient required treatment (epinephrine, corticosteroids, hospitalization).
Isolated non-specific symptoms (nausea, diarrhea, headache, "family history only," or reaction of unknown timing/morphology) are classified low risk and can often proceed directly to oral challenge. Urticaria, pruritus, or flushing without anaphylaxis is moderate risk. Anaphylaxis, laryngeal edema, or recurrent IgE-type reactions are high risk but still testable by an allergist in a monitored setting. Penicillin itself is not directly immunogenic; it becomes so after its reactive β-lactam ring opens and covalently haptenates host serum and cell-surface proteins. The resulting antigenic determinants fall into two classes:

Testing with PPL plus MDM identifies roughly 90–97% of individuals who will react to penicillin skin testing; PPL alone captures a smaller share, which is why full-determinant testing is preferred whenever available.
Step 1 — Percutaneous (prick) test: A drop of each reagent (PPL, MDM, and the challenge drug itself if desired, e.g. amoxicillin) is placed on the volar forearm and the skin pricked through it with a sterile lancet. A histamine positive control confirms the skin is reactive (antihistamine-free, mast cells responsive); a saline negative control excludes dermatographism/false positives. Step 2 — Intradermal test (only if prick is negative): 0.02mL of a dilute reagent is injected intradermally, raising a small bleb, again alongside histamine and saline controls. Read again at 15 minutes using the same wheal-and-flare criteria. A positive result at either step halts the protocol: the patient is classified as skin-test positive, oral challenge is not performed, and the allergy label is retained (and refined with the specific positive reagent documented).
Skin testing is performed by trained personnel with epinephrine, oxygen, and resuscitation equipment immediately available, because intradermal testing — while rare — can itself trigger a systemic reaction, particularly in high-risk (recent anaphylaxis) patients. Patients must be off antihistamines for an appropriate washout period (typically 3–7 days depending on agent, longer for some tricyclics) since these blunt the wheal-and-flare response and can produce a false-negative result. Because skin testing predominantly detects IgE-mediated (immediate, Type I) hypersensitivity, it has no role in evaluating T-cell mediated severe cutaneous adverse reactions (SJS/TEN, DRESS, AGEP) — for these, skin testing is not merely unhelpful, it is contraindicated, since intradermal challenge can theoretically provoke recurrence of a severe reaction.
A negative penicillin skin test is not merely "reassuring" — it is one of the best-validated negative predictive values in all of allergy medicine. IgE-mediated drug sensitization is not permanent. Longitudinal studies (Blanca et al.; Sullivan et al.) tracking penicillin-specific IgE over time found that roughly 50% of previously sensitized individuals lose detectable specific IgE within 5 years, and roughly 80% do so within 10 years of the index reaction. This underlies why remote reactions (occurring many years prior) carry a lower probability of a positive skin test and a correspondingly higher likelihood of successful delabeling — though remote timing alone is never used to skip testing in moderate/high risk patients; it refines pretest probability rather than replacing the test itself.
A negative skin test combined with a tolerated oral challenge reduces the residual risk of an IgE-mediated reaction on subsequent full-dose penicillin exposure to well under 1% — a residual risk lower than the baseline rate of a first-time drug reaction in an unlabeled patient.
Regardless of skin test result, direct clinical tolerance is what ultimately matters, and the graded oral drug challenge is considered the gold-standard diagnostic procedure in drug allergy — it is the only test that reproduces the actual clinical exposure being evaluated. In low-risk patients it can be performed as the sole step (skipping skin testing entirely); in moderate/high-risk patients it is the final confirmatory step after a negative skin test.
Low-risk patients (per history or PEN-FAST) are increasingly challenged with a single, full therapeutic dose of oral amoxicillin, observed for an allergic reaction — this "direct challenge" approach, bypassing skin testing altogether, has been validated in large prospective cohorts with excellent safety and dramatically improves throughput in busy inpatient delabeling programs.
Moderate/high-risk patients (or any patient for whom a more cautious approach is preferred) undergo a graded, two-step challenge: an initial small dose (commonly ~10% of the therapeutic dose) is given and the patient observed for 20–30 minutes for any reaction; if tolerated, the remaining ~90% is administered, followed by a further 30–60 minute observation period. Challenges are conducted in a monitored clinical setting with baseline vital signs recorded, continuous or frequent reassessment during observation, and immediate access to epinephrine and resuscitation equipment — though actual severe reactions are rare when patient selection (history + optional skin test) has appropriately excluded high-risk and contraindicated patients beforehand. Objective findings that terminate the challenge and confirm a positive result include new urticaria, angioedema, wheeze/bronchospasm, hypotension, or anaphylaxis; subjective symptoms alone (flushing, mild GI upset, anxiety-related symptoms) are common, generally benign, and do not necessarily equate to a true allergic reaction — distinguishing the two requires clinical judgment and sometimes a repeat, slower challenge on a different day.
Skin testing detects sensitization — the presence of drug-specific IgE bound to mast cells — but sensitization does not always translate into a clinical reaction upon exposure, and skin testing does not evaluate delayed, T-cell mediated hypersensitivity at all. Removing an inaccurate penicillin allergy label is not simply an administrative correction — it measurably changes the antibiotics a patient receives for the rest of their life. Health-system delabeling programs consistently show a rapid shift toward first-line beta-lactam use, reduced exposure to broad-spectrum alternatives, and downstream reductions in resistant-organism and C. difficile infection rates, making penicillin delabeling one of the highest-yield, lowest-cost interventions available to antimicrobial stewardship programs.
Once a patient has a negative skin test (if performed) and/or has tolerated a graded oral challenge, the allergy is formally removed from the electronic health record — not merely annotated.
